As a courtesy to our patients, our office will file claims for you. Please bring your insurance card and picture ID with you at the time of your scheduled appointment. We will also need to know if you belong to any managed care group, such as a PPO or HMO. It is also required that you update your Patient Registration Form that we have on file every 6 months as well as any change in benefit coverage so that your record is accurate.
Patients are financially responsible for all charges, including any deductibles and/or co-payments. If you have no insurance coverage, payment in full is due at the time of service, unless prior arrangements have been made. For your convenience, we accept cash, personal checks, MasterCard, and Visa.
